Since your screenshot confirms your profile settings are correct, there are usually only two causes.
1. The "Hacker" Trigger
Microsoft OTP emails are often generated based on the location of the device requesting the code, not your profile setting.
- Immediately check your Recent Sign-in Activity.
- If a bot in Russia attempts to login, the system sends the OTP in Russian. If you see any Unsuccessful or Successful attempts from Russia, change your password immediately.
2. The "Toggle" Reset
Sometimes the backend database disconnects from the frontend display shown in your screenshot. You need to force it to update.
- Change your "Language" to English (United Kingdom) and save. Log out. Log back in, and switch it back to English (United States). This forces the server to rewrite your preference file.
3. Check Outlook.com Specifically
Your general Microsoft Account settings (shown in your image) are separate from the email service settings.
- Action: Go to Outlook.com -> Settings (Gear) -> General -> Language. Ensure this isn't set to Russian.
